In this role, you will serve as the primary liaison between healthcare providers and home health teams, ensuring a seamless transition for patients into home-based care.

Responsibilities

• Develop and strengthen relationships with physicians, hospitals and healthcare providers
• Coordinate home health referrals from initial referral through ongoing patient care
• Evaluate patient suitability for home health services
• Act as the key communication link between physicians, caregivers and clinical teams
• Coordinate follow-up appointments and patient care plans
• Ensure physician orders and documentation are completed accurately and efficiently
• Support the delivery of medical equipment and additional patient services where required
• Deliver educational presentations to healthcare professionals on available home health services
• Identify opportunities to grow referral partnerships and increase market presence
• Maintain accurate CRM records and activity reporting

Skills

• Formal clinical training from an accredited institution
• Active professional licence in your state of practice
• Excellent communication, presentation and relationship-building skills
• Strong organisational and problem-solving abilities
• A current driver's licence and access to reliable transportation for local travel
• Previous Home Health experience
• Experience using Microsoft Office and CRM systems
