This position plays a vital role in helping patients with complex chronic conditions receive ongoing care coordination, education, and support between provider visits. The ideal candidate is experienced in patient communication, care coordination, documentation, and working independently in a remote environment. Qualifications: Medical Assistant Certification Minimum 5 years of medical office, care coordination, or chronic care management experience. Strong computer skills and proficiency with EHR systems. Excellent verbal and written communication skills. Ability to work independently in a remote setting. Strong organizational and time-management skills.
