Primary Care Physician| Low patient volume | M-F with 1 remote day per week
Job Description
Primary Care Physician Locations: Dartmouth, Leominster or Springfield, MA Compensation: $275,000–$300,000 annually, based on experience Join a mission-driven healthcare organization delivering comprehensive, community-based care to older adults through a PACE (Program of All-Inclusive Care for the Elderly) model. - Low Patient Volume: Average of 4 patients per day with a panel of ~65
- Team-Based Care: Work closely with an interdisciplinary team including nursing, therapy, social work, and care coordination
- Mission-Focused Practice: Help frail elders remain safely in their homes and avoid nursing home placement
- Professional Support: Strong peer mentorship and leadership engagement
Key Responsibilities
- Provide primary, preventive, urgent, and chronic care to PACE participants
- Conduct initial and biannual assessments and contribute to individualized care plans
- Participate actively in interdisciplinary team (IDT) meetings
- Coordinate specialty care and transitional care services
- Deliver care across settings including clinic, home, and inpatient facilities
- Participate in limited after-hours call rotation
- Maintain timely and accurate clinical documentation
Qualifications
- MD or DO licensed in Massachusetts; DEA required
- Board-certified or board-eligible in Internal Medicine, Family Medicine, or Geriatrics (geriatrics preferred)
