We are hiring social workers to be dedicated care managers for older adults, covered by Medicare, so family can just be family again. We’re looking for social workers passionate, ideally with experience in palliative care, hospice, or geriatrics, who are passionate about making a difference in the lives of older adults and their families. You’d Be a Great Fit If You:

• Live in Massachusetts
• Love creating connection with families and older adults
• Find yourself to be a creative problem-solver and want a job that looks different every day
• Are tech-savvy and excited to work in a fast-paced, early-stage environment
• Hold an LMSW or LCSW (clinical license is a plus but not required); note a NY license is not required
• Have 2+ years of experience in care management, hospice, palliative care, or geriatrics
