About the position Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Registered Nurse (RN) Case Manager in Lynn, Massachusetts. This hybrid role focuses on coordinating care for aging adults living independently at home. It is a community-based nursing opportunity centered around care planning, continuity of care, and service coordination, with no bedside nursing responsibilities required. Responsibilities

• Develop individualized patient care plans
• Coordinate community-based services and ongoing care needs
• Conduct patient visits within community and home settings
• Manage follow-up and care coordination remotely
• Collaborate closely with interdisciplinary care teams
• Support aging adults in maintaining safe independent living

Requirements

• Active Massachusetts RN license REQUIRED
• Minimum 2 years RN experience REQUIRED
• Strong communication and organizational skills REQUIRED
• Comfortable working independently within community settings REQUIRED
• Team-based care approach REQUIRED
• Comfortable with hybrid remote/community workflow REQUIRED
