Job Description

We're looking for a skilled freelancer to design a custom vendor form that will be used by careerzynith to onboard customers, including hospitals, surgery centers, doctor offices, schools, and other healthcare institutions. The ideal candidate will have expertise in form design, proficiency in LaTeX or Adobe Acrobat, and a deep understanding of U.S. business and tax requirements.

Key Responsibilities

As our freelance form designer, you will be responsible for creating a clean, editable PDF template that meets the following requirements:

• Customer Information: Include sections for:

+ Legal name + DBA (Doing Business As) + Billing address

• Contact Details: Include sections for:

+ 2 Accounts Receivable contacts + 3 key contacts + Optional Head Service Person

• Tax and Compliance Info: Include sections for:

+ EIN (Employer Identification Number) + W-9 (Certificate of Foreign Status of Beneficial Owner for United States Tax Withholding) + FDA registration #

• Purchasing, Shipping, and Payment Details: Include sections for:

+ Purchasing information + Shipping details + Payment terms

• Required Documentation: Include a section for:

+ W-9 + Purchase order + Signed form

Deliverables

As our freelance form designer, you will deliver the following:

• A fully functional PDF form template
• Source files (e.g., LaTeX code or design file) for future edits
• Brief instructions on how to use/edit the form

Skills and Experience
