Role Overview: The Epic Analyst - Healthy Planet Population Health will play a key role in implementing, optimizing, and maintaining the Epic Healthy Planet module to enhance population health management. This position requires close collaboration with clinical, operational, and IT stakeholders to align workflows, care coordination, and data-driven strategies that improve patient outcomes. Key Responsibilities:

• Configure, build, and maintain the Epic Healthy Planet module to support population health initiatives.
• Collaborate with clinical and operational leaders on workflows for care management, preventive care, and chronic disease monitoring.
• Analyze population health data and metrics to support targeted care strategies.
• Develop dashboards and reports to monitor performance and quality indicators.
• Assist with upgrades, testing, and change management.
• Stay current with Epic upgrades, functionality, and best practices.

Required Skills & Experience:

• Epic Healthy Planet certification (minimum 2+ years)
• Hands-on experience configuring, building, and maintaining Healthy Planet module (2+ years)
• Experience collaborating with clinical/operational teams (2+ years)
• Strong background in population health data analysis and reporting (2+ years)
• Prior experience in a hospital and/or clinical setting
